How to Obtain Your Records

Immunization Records

Cabarrus Health Alliance (CHA) uses the North Carolina Immunization Registry (NCIR) to maintain immunization records. Most NC healthcare providers can access this registry.

Certified Copies: Provided at the time of immunization. Replacement copies: $5 fee. No charge if needed for school or work requirements.

Request Options:

  • In Person: Monday–Friday, 8:00 AM – 4:30 PM. Bring a valid photo ID.
  • Alternative Methods: Call 704-920-1205 for assistance.

Faxing Records: CHA can fax immunization records to schools or providers at no charge. Call 704-920-1205 to request.

Medical, Dental, or Live Well (Behavioral Health) Records

  • Requires a signed authorization to release information.
    •  For children’s records (under age 18),  a parent/legal guardian must sign the release, however exceptions apply for minor-consented services.
    • Most requests are processed within 5 business days.
  • Fees:
    • Up to $6.50 for personal copies.
    • No charge for transfers to another provider.

Authorization Submission Options:

We offer several convenient options for completing request for records. 

  • In-Person: Bring a valid photo ID.
    • Clinical Services: Kannapolis office, Monday–Friday, 8:00 AM – 4:30 PM
    • Dental: Kannapolis or Concord Office, Monday-Friday 8:00 AM-4:30 PM
    • Live Well: Concord office (363 Church St N Suite 240), Monday–Friday, 8:00 AM – 4:30 PM
  • Through Receiving Provider: The authorization form can be signed at the office where the records are being transferred, then faxed by the receiving office to CHA. 
  • Other Options: If you are unable to sign the authorization in-person, we may have alternative solutions available. Please contact us for assistance:

Form Completion Requests

For FMLA, Physical, or Disability Forms, CHA providers can complete forms for established patients. Please ensure that patient portion of the form has been completed.

After Submission:

  • Completed forms are available for pickup.
  • To fax to a third party, submit a signed Authorization to Release Information.

Important Notes:

  • Must be a current CHA patient (physical exam with provider with in a year)
  • Physical/school health forms require a Physical Exam within the current calendar year.
  •  Allow up to 5 business days for completion.
