How Do I Get Started?
Step 1: Get tested
Journey to a Healthier Me offers an A1c screening at NO COST to you! If you have had an A1c within the past year that was in the prediabetes range, you will not need to be re-tested.
Step 2: Confirm your eligibility
To participate in the program, you must meet the following requirements:
- Be at least 18 years old
- Have a body mass index (BMI) of at least 25 (23 if Asian)
- Don’t know what your BMI is! visit, https://www.nhlbi.nih.gov/health/educational/lose_wt/BMI/bmicalc.htm
- Have no previous diagnosis of type 1 or type 2 diabetes
- Have received a blood test result in the prediabetes range in the past year, OR have been previously diagnosed with gestational diabetes (diabetes when pregnant)
- A fasting blood glucose score of 100 to 125 mg/dl
- An A1c test resulting in a level of 5.7-6.4 percent, or
- Plasma glucose measured 2 hours after a 75 gm glucose load, with a result of 140-199 mg/dl
You cannot participate in the program if you:
- Are pregnant
- Have already been diagnosed with either type 1 or type 2 diabetes
- Have a BMI less than 25 (23 if Asian)
