1. How do I obtain a copy of my immunization record?

You can come by our office between the hours of 8 a.m. to 5 p.m. to pick up a copy. There is a $5 charge for a copy of a shot record. If you would like this faxed to a doctor’s office, school, or day care, we can do this for you at no charge. Please call 704-920-1208 for more information.

2. How do I obtain a copy of my medical records?

You would need to come to our office and sign a release of records. Please bring your photo identification with you. There is a $10 charge for a copy of medical records. Please allow 48 hours to process release.

3. What do I need to do to get my medical records transferred to another provider?

You would need to come by our office between the hours of 8 a.m. to 5 p.m., and sign a release of records. Please bring your photo identification with you. We will need 48 hours to get the records processed to be sent to your then new provider.
You may also choose to sign the release of records at the new doctor’s office that you will be going to. They will then send the release to us. Once we receive the release we will forward the records to them.

5. What is the fiscal year?

The Cabarrus Health Alliance’s fiscal year is July 1 to June 30. CHA, like many other governmental entities, operates on an annual financial (or fiscal) year which is distinct from the calendar year. For example, fiscal year 2013 began on July 1, 2012, and ends on June 30, 2013.
By law, the State of North Carolina and all North Carolina counties and municipalities operate with such fiscal years. The Cabarrus Health Alliance is a local governmental entity.

6. Is the Cabarrus Health Alliance audited?

CHA is audited by an independent external auditing firm each year. The CHA is required by North Carolina General Statutes to have an audit of the general financial statements performed by an independent Certified Public Accountant (CPA).
The CPA issues an opinion that is published with the CHA's annual audited financial statements. As overseer, the state treasurer receives a copy of the CHA's annual audited financial statements. The annual audited financial statements are available for public review online and at the CHA Finance Department.

7. What is a fund balance and the Cabarrus Health Alliance's policy regarding fund balance?

A fund balance is defined as the difference between the assets and liabilities of a fund. The North Carolina Local Government Commission states that fund balance available for appropriation should not drop below 8% of total expenditures.
The CHA has also adopted a minimum fund balance policy for the General Fund which instructs management to conduct the business of the CHA in such a manner that available fund balance is no less than 15% of budgeted expenditures.

8. Does the Cabarrus Health Alliance pay for indirect costs to its subcontractors?

The CHA does not reimburse for indirect costs. The CHA relies on funding from federal, state, and local entities as well as some private foundations to provide services to the citizens of Cabarrus and surrounding counties. No funds are budgeted to cover indirect costs to subcontractors.

9. How does the Cabarrus Health Alliance process surplus property for sale to the public?

10. Does the Cabarrus Health Alliance accept credit and debit cards for payment?

The CHA accepts cash, checks, and most credit and debit cards for payment of services. There is a $25 fee for returned checks. Payment is expected at time services are rendered.
