[ARCHIVED] Community Highlight: MAKING CONNECTIONS

“MAKING CONNECTIONS” is a way of planning and delivering services to your child to help them make smooth and successful moves or transitions from school to adult life.There are three specific areas that transition services target:
•Employment,
•Post-secondary education or training and
•Independent living.

In “MAKING CONNECTIONS”, a School Level Team (SLT) is made up of school staff and caseworkers as well as case managers from various outside agencies who may be of assistance to your child as he/she makes successful transitions in life. They come together to meet with your child, you, your child’s teacher, and anyone else you and your child would like present.
During the SLT meeting, your child is given the opportunity to share his/her strengths and needs and together, the team works to create the best possible plan for success after high school.

MAKING CONNECTIONS helps make the transition out of high school much easier for youth with special needs and their families.
