If you are concerned about the care or services provided to patients/residents/consumers by health care facilities/agencies/homes licensed by Division of Health Service Regulation please contact the Complaint Hotline: 1-800-624-3004 (within N.C.) or 919-855-4500. The Complaint Hotline hours are 8:30am to 4:00pm weekdays, except holidays. Facilities include, but are not limited to nursing homes, adult care homes, acute care hospitals, home care agencies, home health agencies, hospice agencies, etc.
