What it is:
Home Health provides skilled nursing care by Registered Nurses, physical therapy, speech therapy, occupational therapy, medical social services, nursing assistant care, and nutritional counseling in the patient’s home.

Statement of purpose: The purpose of home health is to serve the homebound patient. Home health prevents the unnecessary cost and utilization of hospital or nursing home beds when safe and appropriate care can be given at home. It also teaches people to care for themselves independently if they have a chronic disease such as diabetes.

Who may receive this service: The service is available to all residents of Cabarrus, Stanly, and Rowan counties. Limited service area in northern Mecklenburg county. All services are provided under the orders of the patient’s physician.

How this program is carried out: Patient can be referred by the individual, family, friend, or physician. A registered nurse makes an evaluation visit to assess the patient and develop a plan of care specific for the individual patient. The plan of care is approved by the physician.

How the community benefits: People are able to recover from illnesses and surgeries in their own home where they are happier and more comfortable. In many instances home health can prevent nursing home placement.
