WHAT IT IS:
This program provides case management of pregnant women and children up to age 5.

STATEMENT OF PURPOSE:The purpose of this program is to promote and maintain health; detect health problems; and offer resources, referrals and education to pregnant women and children.

WHO MAY RECEIVE THIS SERVICE: Any pregnant woman who receives Medicaid requesting services or any child up to age 5 with a potential or diagnosed health condition.

HOW THIS PROGRAM IS CARRIED OUT: Any pregnant woman who receives Medicaid or the parent/guardian of a child may request services. Medical providers or other agencies within the community may also refer clients to this program. Contacts are usually made in the clinic or in the home on a regular basis.

HOW THE COMMUNITY BENEFITS: Family Care Coordination helps to ensure that families have access to needed resources.
