What it is: The Logan Community Family Resource Center is a satellite clinic of the Cabarrus Health Alliance located in Concord.  The clinic provides public health services, prenatal care, family planning, TB skin testing, child health and sick care for children, STD, WIC, lab and childhood immunizations.  Free HIV testing is also available.  Interpreter services are available.

Statement of purpose: The purpose of this program is to provide mandated public health services and referrals. Our goal is the protection and promotion of health for our community.

Who may receive this service: Logan accepts persons with Medicaid, Medicare, North Carolina Health Choice for Children and various insurance plans. There is a fee for service for sick care.  Financial eligibility for services is determined by federal guidelines and a sliding scale fee structure based on family size and income.

How this program is carried out:  All services are by appointment only. Physicians, nurse practitioners, public health nurses, nutritionists, and lab workers collaborate as a team to assess health and treat disease/illness. 

Hours of Operation
Health services: Monday - Friday 8:00 am - 5:00 pm. by appointment only.
WIC services: Monday, Wednesday, and Friday 8:00 am – 5:00 pm by appointment only. We are closed the second Friday afternoon of each month.

Closed on New Year's Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day and the day after Thanksgiving and Dec. 24 and 27 2004.
