Healthy Cabarrus Access to Healthcare Task Force
The purpose of the
Access to Healthcare Task Force is to work together to identify health
service needs, to facilitate collaboration among providers to address needs,
to avoid duplication of services, and ultimately to maximize access to care
for the underserved.
The access to
Healthcare Task Force initiatives include:
Prescription
Medication Resources:

Identification of available resources and gaps in the services
·
Coordination of medication assistance services
·
Development and distribution of the Prescription Medication Resources
Matrix
·
Supported local collaboration to seek Medication Assistance grants from
the Health and Wellness Trust Fund
·
Support and assistance with local awareness of Medicare Part D and NCRx
and sign-ups with the Senior Center
Healthcare
Safety-Net Providers
·
The task force supports start-up, community awareness, and referral
networking for healthcare safety-net providers such as the Cabarrus
Community Health Centers that was established in 2006 to provide healthcare
for low-income residents.
Prostate Cancer
Screening Initiative - addressing a community health disparity
·
Served as a planning group for the Cabarrus Health Alliance Prostate
Cancer Screening program
Grants and Funding
support
·
Develop and support collaborative grant initiatives to seek funding for
access to healthcare issues such as Prostate Cancer, Medication Assistance,
Community Health Grants, and the Community Care Plan
Health Care
Legislation
·
Monitor, assess, and support needed health care legislation at the state
and national levels – ex:
Children’s Health Insurance Program, Health Net – Community Care Plan funds,
High Risk Insurance Pool, and Funds for Safety Net Providers
