Frequently Asked Questions for Medical Records

How do I obtain a copy of my immunization record?
You can come by our office between the hours of 8am-5pm to pick up a copy. There is a $5 charge for a copy of a shot record. If you would like this faxed to a doctor’s office, school or day care, we can do this for you at no charge. Please call 704-920-1208.


How do I obtain a copy of my Medical Records?
You would need to come to our office and sign a release of records. Please bring your ID with you. There is a $10 charge for a copy of medical records. Please allow 48hrs to process release.

What do I need to do to get my Medical Records transferred to another provider?
You would need to come by our office between the hours of 8am-5pm and sign a release of records. Please bring your ID with you. We will need 48hrs to get the records processed to be sent to your then new provider.

You may also choose to sign the release of records at the new doctor’s office that you will be going to. They will then send the release to us. Once we receive the release we will forward the records to them.
